Position Description
Thriving Community Health Center is looking to hire a Family Nurse Practitioner in a Primary Care setting in Wilmington, DE!
Providers have the opportunity to make a meaningful difference by caring for patients who may otherwise struggle to access primary care!
About The Group:
- Federally Qualified Health Center (FQHC) focused on delivering high-quality care to underserved and underinsured populations in Delaware.
- Nurse Practitioners care for patients across the lifespan with services that include family medicine, women’s health, chronic disease management, behavioral health, and preventive care.
- Collaborative, team-based environment with integrated services including behavioral health, primary care and dental care

About the role:
- Full-time position - 4 or 5 days/week
- Primary Care position seeing all ages
- Family Nurse Practitioner will work with a collaborative team
- Seeing an average patient load of 18-20 patients/day
Background Desired:
- Board Certified Family Nurse Practitioner
- 1+ years of experience as a Family Nurse Practitioner
- Passionate to work in a community health setting
