Job Details (Locum Tenens Nurse Practitioner – Baltimore, MD)
- Location: Baltimore, MD (virtual and in-person care model; in-person visits in physician offices/dialysis centers as needed)
- Specialty Focus: Nephrology / CKD-focused primary care
- Start Date: March 23, 2026
- Assignment End Date: June 20, 2026
- Duration: 13 weeks
- Schedule: Monday–Friday (full-time, self-managed schedule)
- Shifts per Week: 5
- Shift Duration: 8-hour days
- Hours per Week: 40
- No nights/weekends/on-call: Yes
- Estimated Weekly Pay Range: $3,690–$4,370
Job Requirements (Nurse Practitioner Qualifications)
- Experience: Minimum of 2 years as a Nurse Practitioner (NP)
- Licensure: Current unrestricted NP license to practice in the state (or ability to obtain)
- DEA: Active DEA license or eligible to obtain within 90 days
- Certifications: Current BLS/CPR
- Clinical Background: Experience managing chronic diseases (e.g., CHF/HF, diabetes, hypertension); comfort prescribing for HF, HTN, and DM
- Technology Skills: Comfortable working across multiple EMR/EHR systems; intermediate proficiency with MS Word, Excel, and Outlook
- Preferred: Dialysis/nephrology/primary care experience; value-based care or population health experience (including documentation/coding and health risk assessments); familiarity with palliative care discussions and planning
Responsibilities (CKD Primary Care Nurse Practitioner)
- Provide CKD-focused primary care through a virtual and in-person care model
- Conduct comprehensive assessments, develop evidence-based care plans, and manage medications for complex chronic conditions
- Deliver patient care in appropriate settings (including in-person visits in physician offices/dialysis centers and virtual follow-ups) to support continuity of care
- Collaborate with interdisciplinary clinical partners to coordinate care, close care gaps, and improve outcomes
- Support proactive, longitudinal management aimed at reducing avoidable hospitalizations and readmissions
- Document accurately and efficiently in EMR/EHR systems and adapt to evolving workflows in a new care model
- Educate and engage patients in shared decision-making to improve adherence, self-management, and patient experience
